Record Request Process

What to Know

  • Records are only provided with a release signed by student.
  • The records maintained by Student Accountability & Support are separate from transcripts, which are maintained by the Office of the University Registrar.
  • Records are retained permanently for incidents involving a suspension, dismissal or expulsion.
  • Incidents involving discrimination and sexual assault are handled by the Office of Institutional Equity. They can be reached at 573-882-3880 or equity@missouri.edu.
  • We only have access to the disciplinary conduct records of our students. For academic information you can contact the Office of the University Registrar. For academic conduct, you can contact the Office of Academic Integrity.

Requesting Disciplinary Records

Any student or other agency may request a summary letter of a student’s disciplinary record by submitting the necessary release authorization form from the requesting institution. The completed form should be emailed to the Office of Student Accountability and Support at accountability@missouri.edu, dropped off at the office or mailed to:
